Graduation Requirements Check List

LOCUS is a voluntary program. However, we want to provide you with substantive acknowledgement for your participation in this time-intensive program. If you complete the requirements outlined below, you will graduate as a LOCUS fellow. As a fellow, Cindy Haq or your mentor will submit a letter to the Dean's Office by the fall of your fourth year that will be included in your Dean's letter. You are responsible for providing the necessary information for this letter. You should be documenting all of your LOCUS project activities throughout your participation in the program. This documentation should be submitted by the Spring of your third year. You will also receive a certificate of achievement. Please note: you may not be able to complete the option from List B until your fourth year. You will be considered a LOCUS fellow as long as you complete the requirements.

Fellows complete the following prior to submission of the letter to the Dean's Office:

Year 1: Beginning of Semester 2 New Project Report Submit Form
Year 2: Beginning of Semester 1 Project Update Submit Form
Year 2: Beginning of Semester 2 Project Update Submit Form
Year 3: Beginning of third year Project Update Submit Form
Year 4: Fall of fourth year 2-3 page final summary of project that includes a reflective evaluation of the project process N/A
Ongoing Participate in program evaluation (written and/or focus group) View Options

Choose one option:
1. Present a poster or make a presentation during your second, third or fourth year at one of the following local meetings:

  • LOCUS Fall Retreat
  • Department of Family Medicine (DFM) Fall Statewide Meeting
  • LOCUS Spring Retreat
  • DFM Spring Statewide Meeting
  • .... or at a national meeting (abstract due dates vary)
  • National Conference of Family Practice Residents and Medical Students
  • Society of Teachers of Family Medicine (STFM) Predoctoral Education Conference
  • STFM Annual Meeting
  • Other Meeting of Your Choice
2. Prepare a paper for submission to a journal
3. Select a creative option from the list below (ask for more detailed instructions regarding these options if you are interested):
  • Assemble a LOCUS Project Portfolio: A portfolio is a collection of documents and other forms of evidence of your competencies and achievements, specifically as they relate to your project/program goals and objectives.
  • Prepare an Artistic Reflection Piece: Examples include: a community of ideas mural (illustrating what you have learned about the community your worked with and yourself during LOCUS); a photo journal of your experiences; or a videotape of your experiences during your project.
  • Keep a Personal Journal
