Helping patients develop healthier lifestyles in 2021 through peer support

  • Who? Verona clinic patients motivated to make lifestyle change who have a body mass index (BMI) of 30 or greater.
  • What? Shared medical appointments (group visits) in a virtual format from the Verona Clinic. The group visit framework:
    • Guided stretching/relaxation and light cardio exercise by video
    • Sharing in a healthy meal prepared by Mad City Chefs
    • Educational topics (SMART goal setting, reading food labels, practical snacking, healthy eating out tips, mindful eating, & more)
    • Small group facilitated SMART goal setting​
  • When? 2nd and 4th Mondays 6:00-7:00 PM (3/8, 3/22, 4/5, 4/19, 5/3, 5/17 )
  • Where? these will be virtual visits facilitated through the UW Health Verona Clinic (608) 845-9531; 100 N Nine Mound Rd, Verona, WI 53593
  • Why? There are many benefits of challenging yourself to a more healthy lifestyle!

Nutrition Support

  • Community nutrition support
    • Kara Hoerr Nutrition
      • One free initial (60-minute) coaching session to all participants
      • Virtual accountability through a food tracking app (similar to MyFitnessPal) with encouragement and tips provided for the entire duration of the program for $100 (50% discount)
    • Festival Foods’ Mealtime Mentors
      • From making a meal in 20 minutes to getting your kiddos to eat more fruits and veggies to helping you transition to a new way of eating, Festival Foods’ team of registered dietitians is here to support you. Ask the Mealtime Mentors your food and nutrition questions.
      • Are you starting a new eating pattern or following a restricted diet? The Mealtime Mentors have resources to help make it easier.
    • Hy-Vee Fitchburg
      • Find balanced meal ideas with healthy side suggestions and complete nutrition facts. Just click Print & Shop at the bottom to select which meals you want on the menu this week. It’s that easy.
      • This link will lead you to Hy-Vee food service items as well as a low-sodium and gluten free product list. For more assistance, reach out to dietitian Kacie.
      • Personalized shopping assistance/nutrition store tour (FREE). Each tour can be customized, with a Q&A along the way. Most tours last one hour.
  • New for 2020! Healthy meal kits prepared by Mad City Chefs delivered once weekly on Mondays to the UW Health Verona Clinic. Participation in this is optional for an additional cost of less than $7 per serving

Fitness Support

  • Videos: Theraband Exercises Playlist
  • Guided stretching & relaxation at each group visit
  • Participants are expected to exercise 2 times per week over the 20 week challenge OR walk 35,000 steps per week (5,000 steps per day)
  • Consider the following apps to help you support you while you strive to meet your fitness and lifestyle goals
  • You can also use a fitness and food log to track your food and beverage intake and log your steps and activity.
