Bio
Sarah Zelazoski, MD (she/her/hers) joined DFMCH in 2025 as a resident. Growing up in a bilingual household with Peruvian roots, Zelazoski developed an early appreciation for culture, empathy, and individualized care. Her path to family medicine was shaped by years of community engagement—from working as a caregiver in Middleton, Wisconsin, to volunteering at free clinics and organizing health fairs in underserved neighborhoods in Albany, New York. Her experiences deepened her commitment to reproductive health, adolescent medicine, and behavioral health.
Zelazoski is drawn to family medicine for its emphasis on long-term patient relationships and personalized care. During medical school at Albany Medical College, she led quality improvement initiatives to increase mental health screenings and mentored first-year students to promote wellness and academic success. She also worked as a Clinical Research Assistant at the Medical College of Wisconsin, where she gained firsthand experience balancing clinical care with research and compassion.
Outside of medicine, Zelazoski enjoys baking elaborate cakes, working out, reading historical fiction, hiking with her dog, watercolor painting, and exploring local farmers markets.
