Intended Audience: All are welcome. The DFMCH Forum is a multidisciplinary and interprofessional conference series designed to highlight advances in clinical care, health equity, primary care research, medical education, and public, community and population health. The DFMCH Forum qualifies for a maximum of 1AMA PRA Category 1 Credit.

Kym Ashford is a dedicated Community Health Worker Supervisor for the ConnectRx WI program, bringing decades of experience working within health systems to eliminate health disparities. Throughout her career, Kym has held various roles focused on improving access to healthcare and addressing the unique challenges faced by underserved communities.

In her current role, Kym oversees initiatives aimed at improving birth outcomes and addressing the social determinants of health impacting Black birthing persons in Dane County. Through ConnectRx, Kym works directly with communities to address critical needs, from healthcare access to social services, ensuring better outcomes for those most at risk.

Kym’s unwavering commitment to advancing health equity drives her work to empower marginalized communities and promote lasting, positive change in the healthcare system.

Adrian Jones serves as the Director of Community Health Improvement in the Office of Population Health at UW Health. Since joining UW Health in 2018 as the program manager for Community Health Improvement, Adrian has drawn on more than 15 years of experience in public health and over a decade in the nonprofit sector.

In her role at UW Health, Adrian oversees the triennial development of the Dane County Community Health Needs Assessment and leads the creation of UW Health’s Community Health Implementation Strategy. She directs a portfolio of initiatives that bridge clinical care and community-based programs, addressing social determinants of health and improving health outcomes in alignment with prioritized community needs. Through multi-sector collaboration, Adrian was instrumental in helping Dane County’s major health systems prioritize reproductive justice and injury prevention as key health focus areas.

Adrian is actively involved in numerous organizations, including serving on the boards of directors for Embolden Wisconsin and the Foundation for Black Women’s Wellness. She is a member of the Black Maternal and Child Health Alliance of Dane County and the African American Opioid Coalition and serves as the Southern Regional Representative on the Wisconsin Public Health Association Board of Directors. Additionally, she co-chairs the Wisconsin Public Health Association’s Racial Equity Committee, where she advocates for policies and educational opportunities that advance racial equity and public health justice.

Adrian holds a dual Bachelor of Arts degree in Gender & Women’s Studies and Afro-American Studies, with an emphasis in sexuality education, from the University of Wisconsin-Madison, as well as a Master of Public Health degree from Tulane University.

Midwest Physician Writers Retreat

Midwest Physician Writers Retreat

April 25-27, 2025 | Madison, WI

Registration Opens: Friday, December 20, 2024 at 8 AM

The Midwest Physician Writers Retreat is a new creative writing conference for physicians and medical trainees set on the shores of Lake Mendota, at the University of Wisconsin – Madison’s Pyle Center. Our hope is to develop a space for reflection, writing and community.

The schedule will begin with a Friday evening welcome reception. Saturday will include guided writing sessions, a plenary talk by Dr. Bruce Campbell, advice about publishing creative work, and structured time to write. Saturday evening, we will host an open-mic event where you can share your writing in a public forum. Sunday morning, we will have additional guided writing sessions and the opportunity to workshop your work with a small group of peers.
