  • Who? Verona clinic patients with type 2 diabetes, a body mass index (BMI) of 30 or greater, and Quartz insurance who are motivated to make lifestyle changes
  • What? Shared medical appointments (group visits) in a virtual format from the Verona Clinic. The group visit framework:
    • Movement and mindfulness activity
    • Eating healthy meals together prepared by BeneBlends, Crisp & Green, Fit Fresh, Forage Kitchen, Sunn Café, and Tropical Smoothie Cafe
    • Educational topics (SMART goal setting, reading food labels, practical snacking, healthy eating out tips, mindful eating, & more)
    • Small group facilitated SMART goal setting​
    • Team support: physicians, nutritionists, behavioral health, pharmacists, physical therapists
  • When? In-person once monthly on Tuesdays 4:30-6:00 PM (1/16, 2/20, 3/19, 4/16, 5/21, 6/11). Virtual check-ins once monthly on Mondays 12:30-1:15 PM (2/5, 3/4, 4/1, 5/13, 6/3).
  • Where? UW Health Verona Clinic: 100 N Nine Mound Rd, Verona
  • Why? Healthy lifestyle changes, including healthy eating and activity, can help you manage your diabetes and help achieve your weight goals

    • Healthier menu items can be found by exploring nutrition information before going to your favorite restaurants. Ask for healthy substitutions when available, such as Splenda in your next smoothie.     • Walk 35,000 steps per week (5,000 steps per day)
    • Theraband Exercises
