Summary of Study:  The purpose of this study is to identify underlying reasons for declining enrollment in the CenteringPregnancy Protocol despite data to suggest improved birth outcomes from receiving prenatal care in this way, and to improve marketing materials and subsequently increase enrollment in the program.

Study Design: The protocol for implementation of this survey will be to provide the self-administered survey to patients of the correct gestational age at the Check in desk.  An alert will be created in the medical record such that these patients will be identified for registration staff and the survey can be administered to the correct people.  Patients will then complete the survey in the waiting room and return the survey box to a secure drop box before the providers see them.  The survey should take no more than 5 minutes to complete and will only be completed by patients one time.
